Emergency Record
Each year, we ask parents to provide information as to whom we should contact in any kind of illness or emergency.

Please carefully check the form that is sent home each year to correct any incorrect information and provide information that is missing. The contact person should live in close proximity to the school to ensure prompt care of your child. Parents’ telephone numbers should be kept current.

Please use this form if your contact information should change during the school year.
